Welcome to my page! My name is Stacie McNabb and I have been a nurse for 11 years. I have spent most of my career as a Neonatal Intensive Care Nurse. This year, I decided it was time for a change, so here I am! I am very much looking forward to a wonderful school year!
Prescribed medications can be given at school if need be. A signed authorization form by BOTH the parent and physician MUST be provided with the medication for it to be given. The medication should come in the original bottle with a label that matches the Doctor's order.
All "as needed medication" (i.e. Tylenol, Motrin) must be brought by the parent in an unopened bottle. The parent MUST sign a form allowing the nurse to administer the medication. This authorization form is only good for 10 days, if need to be kept in clinic longer, a doctor's note must be provided.
